Insurance: Fees

Lab Fees:

Meridian Women's Health contracts out their lab and pathology services. Laboratory Corporation of America and Northwest Pathology bill independently for their lab and pathology services. Medical Records Fees:

Meridian Women's Health provides copies of patient's records upon written request. As a courtesy to our patients and other physicians, Meridian Women's Health provides copies of records to physicians' offices free of charge. If the patient should request a copy of their records there is a nominal copying fee. You may call our office to receive a copy of these fees 206-368-6644.

Other Fees:

Many insurance plans today have co-payments for physician visits. Payment is required before each visit. Meridian Women's Health has a late cancellation/no show fee of $50.00. We require 24 hour notice for all cancellations. Please contact our insurance coordinator at 206-368-6644 if you need further information. We also encourage you to call Northwest Hospital & Medical Center (206-368-6440) to confirm they are participating with your insurance company, before your surgery or delivery.
